Objective
To create a survey to collect data on skills of different people coming to your program centre. This page explains how to quickly setup a survey style form from scratch.
☞ There is also the option of one-click install of Sample Health Project from the Structure > Hierarchy page. Explanation of this project is available on the same page in the Help panel. This is the quickest way to get started on DICEFlow.

1.Project Structure
Project with Levels and Roles are defined. Example shown above
2.Locations added
As per levels defined, Locations are added
3.Users configured
At least, one user is added at the field level.

4.Add Workflow
5.Add Open status
You can add multiple 'Open' statuses. For simplicity sake, we are adding only one. There must be at least one 'Open' status defined
6.Add Closed status
You can add multiple 'Closed' statuses. For simplicity sake, we are adding only one. There must be at least one 'Closed' status defined
7.Add fields to be collected
You can choose different types of fields as per your need
8.One click generate
Generate the Form by using the one-click generate button
9.Flowchart
Shown here is the flowchart post generation. Some validations are included.

Here is how it looks on the Mobile app after you login as the user with the required permissions. Please give WRITE permissions to the Role who needs to initiate this workflow from the Structure > Permissions page
